Individual and Family Therapy: 

$175 per 50 minute session

Initial Intake: 

$225 for 50 minute session
 

Payment:

Payment is due at the beginning of each session; unless otherwise negotiated. I accept all forms of payment.

Insurance:

I currently accept the following insurances as an In-Network provider.  Please check with your insurance company for your specific plan to determine if services are covered and what your deductible, co-pay, or expense may be.

First Choice.

​

I accept all other major insurance companies as an Out-Of-Network provider. Please check with your insurance company to determine your out-of-network benefits. It is likely your insurance company will cover a portion of the cost.

When speaking with your insurance provider, you may consider asking the following:

*Do I need prior approval/referral from my primary care physician?
*How many outpatient mental health sessions are covered per year?
*Is family therapy covered? If so, is a mental health diagnosis for one member required for reimbursement?
*What is my deductible and has is been met?
*What is my reimbursement rate for an out-of-network provider?
*Where can I find the form necessary to submit my own medical claim for reimbursement?

Cancellations:

Please provide 24 hours notice if you need to cancel or reschedule an appointment to avoid being charged for the session. Late cancellations (within 23-0 hrs of scheduled session) will be charged half the session fee. Exceptions are made for emergencies at the discretion of the provider.

 

If you will be more than 15 minutes late for an appointment, please call to reschedule. 

No call/No show:

Full session fee is charged for missed sessions without cancellation.
